Toripalimab (trade name: Tuoyi) is the first domestically produced PD-1 immune checkpoint inhibitor approved for market in China, developed by Junshi Biotechnology. It reactivates T cells to kill tumors by blocking the PD-1 pathway. Currently, it has been approved for 9 indications in China, covering multiple types of cancer such as melanoma, nasopharyngeal carcinoma, lung cancer, and kidney cancer.
1、 Core indications
melanoma
Second line and above treatment for advanced melanoma (first approved indication).
nasopharyngeal carcinoma
Third line and above treatment for recurrent/metastatic nasopharyngeal carcinoma.
Combination chemotherapy is used as first-line treatment for recurrent/metastatic nasopharyngeal carcinoma.
Monotherapy is used for nasopharyngeal carcinoma after platinum based chemotherapy failure.
Urothelial carcinoma
Localized advanced or metastatic urothelial carcinoma after platinum based chemotherapy failure.
Non small cell lung cancer
Combination chemotherapy is used for perioperative treatment of resectable stage IIIA-IIIB non-small cell lung cancer (neoadjuvant+adjuvant).
Combination chemotherapy is used as a first-line treatment for advanced non squamous non-small cell lung cancer that is gene negative.
esophageal squamous cell carcinoma
Combination chemotherapy is used as first-line treatment for advanced esophageal squamous cell carcinoma.
renal cell carcinoma
Combination therapy with Axitinib for first-line treatment of medium to high risk unresectable or metastatic renal cell carcinoma (the first approved immunotherapy for renal cell carcinoma in China).
small cell lung cancer
Combination therapy of etoposide and platinum for first-line treatment of extensive stage small cell lung cancer (the world's first PD-1 inhibitor to achieve dual primary endpoints of OS and PFS in a phase III study of this indication).
2、 Usage and dosage
Recommended dosage
Fixed dose: 240mg, intravenous infusion, once every 3 weeks (current mainstream protocol).
According to the weight plan: 3mg/kg, intravenous infusion, once every 2 weeks (some cancer types/early instruction manual plan).
Route of administration
Intravenous infusion, the initial infusion time should not be less than 60 minutes, and if well tolerated, it can be shortened to 30 minutes later.
Treatment continues until disease progression or intolerable toxicity occurs.
Special reaction handling
If there is temporary tumor enlargement or new small lesions, but the clinical symptoms are stable or relieved, treatment can continue until confirmed progression.
